The PACOP Profile Collection embeds routine holistic assessment of every resident so that no-one is missed. It also facilitates identification of deterioration and the need for palliative care (when to start the PACOP Outcomes Collection)

Your exclusive Profile Collection Education Resources

We’ve got your tools and education resources ready to go!

All residents in your aged care home will be routinely holistically assessed using the PACOP Profile Collection. It will facilitate the early identification of palliative care and end-of-life care needs and deterioration, it also informs comprehensive care planning to respond to identified needs.

Here you will find the tools and resources to support the use of the PACOP Profile Collection to provide comprehensive care.

PACOP Profile Collection - Education Resources

Successful implementation of the PACOP Profile Collection requires a whole team approach which is underpinned by education and support. Quality education is essential for quality care, however we acknowledge that ‘time’ for staff working in aged care is precious. Therefore we have created a range of resources to choose from depending on time, some are comprehensive and others have been designed to provide maximum benefit in limited time.

In this section you will find a wide range of educational resources including:

  • comprehensive clinical guides,

  • PowerPoint presentations,

  • single page education resource sheets,

  • clinical scenarios and

  • other in-service resources.

  • The PACOP YouTube Channel also provides a playlist that includes a range of brief education presentations called ‘Clinical Bites’ (all <10 minutes duration).

PACOP Profile Collection - Tools

The PACOP Profile Collection tools capture information in one place which helps the clinician understand changes in the resident’s overall physical, cognitive and psycho-social and spiritual well-being, as well as the well-being of the resident’s family and/or primary carer. In this way, the Profile Collection clinical assessments help identify residents who are deteriorating and/or may benefit from palliative care.
