Section 3 – Missing clinical items
This section includes a list of clinical items where the value was missing from the phase extract (i.e. it was left blank) when a response was required.
·Check any phases with missing assessments and correct where possible
·Identify if any items are missed consistently – this may indicate issues with how assessments are carried out or how assessments are recorded in your IT system
