Admit to ACH/profile collection

The Change of team status is used only if you home has teams. This allows you to record when a resident has changed from one team to another within your home.

The teams function is only used by some aged care homes and the teams need to be setup in consultation with PACOP. If you are unsure if you need to enter team information, please contact PACOP at pacop-data@uow.edu.au

Note

The Cease Outcomes, return to profile status is only used for residents that are remaining in the aged care home. If the resident is discharged, please use the ‘Discharged from ACH’ status and if the resident dies please use the ‘Death’ status.

Discharge Discharged from ACH

The Discharged from ACH status is used when the resident is discharged from the aged care home. This status is used for residents in both the Profile and Outcomes collections. For residents ending the profile collection, this date can be found on the admission and discharge details tool, next to the words ‘Date of discharge from the aged care home’. For residents ending the outcomes collection, this date can be found on the Palliative Care Episode Start and End of details tool and is the date entered next to ‘What date did the resident cease the outcomes collection’.

On the Status screen, click the ‘Add status’ button in the top right corner of the screen.

The following screen 'Add new status' data entry form will appear. First select the status you wish to enter in the drop down box next to ‘Status type’. Once you have selected the status type, any addition information that needs to be entered will appear below.

Tip

The ‘Now’ button next to the Date and time data entry field is a helpful button to auto-fill the current date and time if data is being entered in real time by your aged care home.

Note

A resident can only return from leave if the first have a ‘Start of leave’ status. i.e. a resident cannot return from leave if they have not left on leave.

Cease Outcomes, return to Profile

The Cease Outcomes, return to Profile status is used if your aged care home is participating in the outcomes collection.

Note

The Cease Outcomes, return to Profile status should only be used for residents that are still living in your aged care home but not longer participating in the Outcomes collection. If the resident has died a Death status should be completed instead. If the resident has been discharged, a Discharged from ACH status should be completed instead.

Death

The Death status is used for residents in both the Profile and Outcomes collection. This information is found on different tools depending on which collection the resident is currently in.

Note

The Death status must always be the last status entered for a resident. No further status can be entered once a Death status has been entered into palCentre.
