The positive and unique identification of a patient: the Patient Identifier ensures that information recorded at each level (e.g. service and pathway) can be associated with that individual, and also allows tracking of the patient through different episodes of care at a pain management service. This number must be used at all times when recording patient, episode, service, pathway and patient -reported outcome level information in epiCentre.
