The Outcomes Collection is underpinned by routine point of care clinical assessment and monitoring of residents’ palliative care needs, symptoms and distress. It includes the use of an evidence-based protocol to support clinicians to assess, respond and plan care appropriately (PACOP ARP Protocol).
