The PACOP Profile Collection tools capture information in one place which helps the clinician understand changes in the resident’s overall physical, cognitive and psycho-social and spiritual wellbeing, as well as the wellbeing of the resident’s family and/or primary carer. In this way, the Profile clinical assessments help identify residents who may benefit from palliative care.

There are three tools that capture this information, these tools include:

  1. Admission & Discharge/death details (the ACH admission ‘bookends’)

  2. Profile Three Part Clinical Assessment (A: face to face, B: desktop, C: action plan)

  3. Deteriorating Resident Tool
