PACOP Profile Collection assessments

The PACOP Profile Collection assessments capture information in one place which helps the clinician understand changes in the resident’s overall physical, cognitive and psycho-social and spiritual wellbeing, as well as the wellbeing of the resident’s family and/or primary carer. In this way, the Profile clinical assessments help identify residents who may benefit from palliative care.

There is one form and two clinical assessments that capture this information, these include:

  1. Admission & Discharge details (the ACH admission ‘bookends’)

  2. Profile Three Part Clinical Assessment (A: face to face, B: desktop, C: action plan)

  3. Deteriorating Resident Tool
