PACOP Outcomes Collection - Education resources

The PACOP Outcomes Collection is for use with residents when the need for palliative care has been identified.

A good understanding of the Outcomes Collection will be provided through reading the Comprehensive Clinical & Assess, Respond & Plan Guides. However we acknowledge that ‘time’ for staff working in aged care is precious. Therefore, we have created a range of resources to suit the needs and roles of your staff, some are comprehensive and others have been designed to provide maximum benefit in limited time. The PACOP YouTube Channel also provides a playlist that includes a range of brief education presentations called ‘Clinical Bites’ (all <10 minutes duration).

The education is based largely on the PHASE (the only new tool introduced) and the Assess, Respond & Plan protocol for clinical staff and the SAS for care staff.

The five assessments included in the PACOP Outcomes Collection Assessment tools used on a regular basis include the:

  1. Symptom Assessment Scale (SAS)

  2. Problem Severity Score (PSS)

  3. Australia-modified Karnofsky Performance Status (AKPS)

  4. Resource Utilisation Groups-Activities of Daily Living (RUG-ADL)

  5. Palliative Care Phase (PHASE)
